Forty years ago, the Challenger space shuttle disintegrated just after lift-off, killing its crew and changing the history of spaceflight.

It's now been 40 years since New Hampshire school teacher Christa McAuliffe died when the space shuttle Challenger exploded.

Disturbingly, Dr Kerwin noted in his report that the force of the blast was insufficient to kill or even gravely injure those aboard.
